I hereby authorize Starmark Financial LLC to initiate recurring or one-time electronic debit entries to (or to otherwise cause funds to be withdrawn from) my accounts at depositories below or the credit/debit card identified by me. There is no charge for any and all recurring transactions authorized herein. The debits are initially made from the bank account, credit card and/or debit card identified by me and may also be made from other bank accounts, credit cards and/or debits cards of which I give Starmark Financial LLC notice by fax, e-mail, or in writing. The debits are to be made in the amounts and frequency and on the dates identified by me or may be made for other amounts, frequencies, or dates, of which I give Starmark Financial LLC notice by telephone, fax, e-mail, or in writing. This authorization shall remain in full force and stays in effect until Starmark Financial LLC has received written notification from me of its termination in such time and in such manner as to afford Starmark Financial LLC a reasonable opportunity to act on it I may give oral notification of its termination as least 3 business days before the scheduled date of the transfer, but I must also give written notice of termination of this authorization within 14 days of oral notification for the oral notification to be binding. I represent that I am authorized to enter into this arrangement or one-time payment for all persons who own or are authorized to access any of my accounts and that such persons will be bound by the terms of this contained herein.
